Guidance for Inquiries Regarding Patient Care

All of the following must be true:

  1. You are an EMS clinician, and
  2. You provided direct care to the patient, and
  3. You provided patient care as part of your employment by an agency that does contract with UW for medical direction, and
  4. The purpose of your request is quality assurance.

Contact your agency’s Service Director or Medical Director directly.

OR

All of the following must be true:

  1. You are an EMS clinician, and
  2. You provided direct care to the patient, and
  3. You provided patient care as part of your employment by an agency that does not contract with UW for medical direction, and
  4. The purpose of your request is quality assurance, and
  5. Your patient was transported to UW Health.

We cannot assist with inquires for patients transported to other hospitals.

Our medical directors will NOT provide any information about patient care for any other purposes.


All other record requests must be made through the medical records department of the appropriate hospital (if seeking hospital records) or from the EMS agency directly (if seeking EMS records) with consent from the patient or their legally authorized surrogate decision-maker.

There are absolutely no exceptions.

Emergency Medical Services is a section in the Division of Prehospital Medicine at the BerbeeWalsh Department of Emergency Medicine, which is a unit of the University of Wisconsin School of Medicine and Public Health and affiliated with the University of Wisconsin Hospitals and Clinics (UW Health).
